We have created 2 initial ways for you to give us some feedback:
Service Provider Experience Survey
This short survey is about capturing your experience with the Waterloo Wellington Local Health Integration Network (WWLHIN) addictions and mental health system as service providers.
Anyone can take this survey, including leaders and direct service staff in addictions and mental health services, hospital service providers, police, or someone providing informal supports. You don’t need to be delivering WWLHIN funded services to take the survey.
Making Connections as Direct Service Staff Survey
The Making Connections survey is designed to understand how direct service staff share knowledge about addiction/mental health and get help with work related things.
This survey is designed to be taken by direct service staff providing addictions and mental health services in the Waterloo Wellington areas. You don't need to be delivering WWLHIN funded services to take this survey.
